If you meet our eligibility requirements, print and fill out the Application for Assistance. Please submit via the postal service, including pertinent documentation

Please be patient through the application process; we have a large pool of applicants and it takes time for us to review your situation, raise funds to meet the need, and locate volunteers/contractors to do the work.
